| 1 | 2.5 mg | 1 to 4 | week 1 | Starting dose |
| 2 | 5 mg | 5 to 8 | week 5 | Maintenance dose |

| 6 | 15 mg | 21 onward | week 21 | Maximum dose |
Dates are the minimum spacing in the SmPC, laid on a calendar, not a recommendation to step up. Your prescriber sets the pace, and a step can be delayed or held at any dose, for example while side effects settle.
Quick answer: The UK Mounjaro Summary of Product Characteristics (SmPC) starts at 2.5 mg once weekly for 4 weeks, then 5 mg, and allows increases of 2.5 mg after a minimum of 4 weeks on each dose, through 7.5, 10 and 12.5 mg, to a maximum of 15 mg. At that minimum pace 5 mg begins at week 5 and 15 mg at week 21. A missed dose can be taken within 4 days, and the injection day can move as long as 3 days separate two doses. Each KwikPen holds four weekly doses of one strength, so one pen is one step.
Educational summary of the UK SmPC, not medical advice. The SmPC sets the minimum time at each step; your prescriber sets your actual schedule, which can be slower at any point. Never change a dose or step up on your own.
What is the Mounjaro dosage chart?
Mounjaro (tirzepatide) starts at 2.5 mg once weekly and moves up in steps, each held for at least 4 weeks. At the minimum pace the SmPC allows, the top dose of 15 mg is reachable at week 21, and the first maintenance dose at week 5.

What does the Mounjaro UK SmPC say about missed doses and changing your injection day?
If a Mounjaro dose is missed, the SmPC says to take it as soon as possible within 4 days of the missed dose. If more than 4 days have passed, skip it and take the next dose on the regular day.
The day of the weekly injection can be changed if necessary, as long as at least 3 days separate two doses.
Strengths. Mounjaro KwikPen, a multi-dose pre-filled pen holding four weekly doses of one strength: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg, or 15 mg.
How is Mounjaro prescribed for weight loss in the UK?
In the UK, Mounjaro is licensed by the MHRA for weight management and is available two ways. On the NHS in England it is being introduced in phases through GP and primary care services from 23 June 2025, starting with the people NICE and NHS England identify as having the highest clinical need, alongside at least 9 months of diet and behaviour support. Privately, registered online pharmacies and clinics prescribe after a consultation in which they must verify your weight, height or BMI. The schedule on this page is the same in both routes because it comes from the licence, not the prescriber.
Each KwikPen holds four weekly doses of one strength, which is one step at the minimum pace. If you stay on a strength for longer, that is a second pen of the same strength, not a change to the schedule.
How does the four-dose KwikPen fit the schedule?
Each Mounjaro KwikPen contains four doses of one strength, dialled and injected once a week, so a pen lasts the four weeks the Summary of Product Characteristics (SmPC) requires at each step. Moving up means the next strength of pen; staying longer at a dose means another pen of the same strength. There is no measuring or unit conversion: the pen delivers its fixed dose.
The pen can be kept out of the fridge (not above30°C) for up to 30 days after first use, per the Summary of Product Characteristics (SmPC), which is also roughly how long a four-dose pen lasts at one dose a week.
What are the Mounjaro maintenance doses?
The Summary of Product Characteristics (SmPC) names 5 mg, 10 mg and 15 mg as maintenance doses, with 7.5 mg and 12.5 mg as the steps between them. 15 mg is the maximum, not a target: your doctor or prescriber decides which dose you stay on, and staying longer at any step is allowed.
FAQ
How long do you stay on 2.5 mg of Mounjaro?
Four weeks, per the Summary of Product Characteristics (SmPC). 2.5 mg is the starting dose, and the maintenance doses are 5, 10 and 15 mg; after four weeks the schedule moves to 5 mg, and your prescriber can keep you on 2.5 mg longer if needed.
What is the highest dose of Mounjaro?
15 mg once weekly, reachable at week 21 at the Summary of Product Characteristics (SmPC)'s minimum pace: 2.5 mg for four weeks, then 5, 7.5, 10 and 12.5 mg for at least four weeks each. Your prescriber can keep you at any step for longer, and 5 and 10 mg are maintenance doses in their own right.
What if you miss a Mounjaro dose?
Take it as soon as possible within 4 days of the missed dose. If more than 4 days have passed, skip it and take the next dose on your regular day, keeping at least 3 days between doses.
Can you get Mounjaro for weight loss on the NHS?
Yes, in phases. NHS England began offering Mounjaro for weight loss through GP and primary care services on 23 June 2025, starting with people with the highest clinical need, and eligibility widens by cohort over the following years. Many people are prescribed privately instead; the dose schedule is identical.
Is Mounjaro the same as Zepbound?
They are the same molecule, tirzepatide, made by the same company. Zepbound is the name used in the United States for weight management; it is not marketed in the UK or Australia, where the weight-management licence belongs to Mounjaro. The dose schedule is identical.
Are the dates in the planner a recommendation to step up?
No. They are the minimum spacing the SmPC requires between steps, laid on a calendar from your first injection date. The SmPC sets a floor, not a schedule: your prescriber decides whether and when you move up, and staying longer at any dose is allowed.
Is this dosage chart free, and do I need to sign up?
Yes, it is free, and no sign-up or email is required. Pick the medication, enter your first injection date, and the schedule updates instantly; there is a print button for the fridge or the clinic. The GlucoPal iOS app it comes from is also free to download.
