Quick answer: mg = units ÷ 100 × concentration (mg/mL), and units = mg ÷ concentration × 100. On a U-100 insulin syringe 100 units is 1 mL, so 25 units is 0.25 mL: 2.5 mg at 10 mg/mL, 5 mg at 20 mg/mL, or 0.625 mg at 2.5 mg/mL. Going the other way, 1 mg is 10 units at 10 mg/mL and 40 units at 2.5 mg/mL. Without the concentration there is no answer, which is why the vial label comes first.

How many milligrams are in a unit?

A unit is a volume, one hundredth of a milliliter, so the milligrams in it are set by the concentration of the liquid: mg = units ÷ 100 × mg/mL. At 10 mg/mL each unit holds 0.1 mg; at 2.5 mg/mL each unit holds 0.025 mg. The converter above takes any concentration, and the quick-reference table shows common unit counts at whatever you enter.

Reversing it gives units from milligrams: units = mg ÷ mg/mL × 100. Both directions are the same relationship, and the concentration is the only number that changes between one vial and the next.

Where is the concentration on your vial?

Compounded GLP-1 vials print a concentration in mg/mL, or a total and a volume that give the same thing: 5 mg/2 mL is 2.5 mg/mL, 34 mg/2 mL is 17 mg/mL. If your pharmacy dispensed a powder vial with reconstitution instructions, the concentration is the mg on the vial divided by the mL of bacteriostatic water the instructions specify, and adding more water lowers it. Switch the converter to “I mix a powder vial” to enter it that way. That mode is for prescription vials, not research-use products, which are not medicines.

Compounded semaglutide and tirzepatide are a US-only option and are not FDA-approved. The FDA has received reports of dosing errors with compounded injections, including confusion between units and milligrams, so a conversion is worth checking twice and confirming with the pharmacy.

Which insulin syringe holds the draw?

U-100 insulin syringes come in three barrels: 30 units (0.3 mL), 50 units (0.5 mL), and 100 units (1 mL). The smaller barrels have finer markings, often half-unit lines, which matters for small semaglutide doses; the 1 mL barrel is needed for larger tirzepatide draws. A draw above 100 units does not fit any single insulin syringe, and the converter flags it. The FDA-approved pens and the Zepbound single-dose vials do not involve any of this: the pens set the dose for you, and a Zepbound vial is always 0.5 mL, drawn in full.

How many mg is 1 unit?

One unit on a U-100 insulin syringe is 0.01 mL, and the milligrams in it depend on the concentration: 0.1 mg at 10 mg/mL, 0.2 mg at 20 mg/mL, 0.025 mg at 2.5 mg/mL. There is no fixed mg-per-unit without a concentration.

How many units is 1 mg?

1 mg is 10 units at 10 mg/mL, 5 units at 20 mg/mL, 40 units at 2.5 mg/mL, and 20 units at 5 mg/mL. Divide the mg by the vial's mg/mL and multiply by 100.

How many units is 2 mg?

2 mg is 20 units at 10 mg/mL, 10 units at 20 mg/mL, 80 units at 2.5 mg/mL, and 40 units at 5 mg/mL.

How many mg is 10 units?

10 units is 0.1 mL. That holds 1 mg at 10 mg/mL, 2 mg at 20 mg/mL, and 0.25 mg at 2.5 mg/mL. Multiply the mL by the concentration on your vial.

How many mg is 50 units?

50 units is 0.5 mL: 5 mg at 10 mg/mL, 10 mg at 20 mg/mL, 1.25 mg at 2.5 mg/mL. A Zepbound single-dose vial is also 0.5 mL, and its strength is printed on the vial.

Is a unit the same on every insulin syringe?

On every U-100 syringe, yes: 100 units per mL, whether the barrel is 30, 50, or 100 units. The barrels differ in capacity and in how fine the markings are (half-unit marks on the small syringes), not in what a unit means. U-40 and U-500 syringes exist for other insulins and use a different scale; they are not used for GLP-1 vials.
